  • Improve adherence to evidence-based clinical guidelines for geriatric conditions in practice.
  • Develop evidence-based plans to diagnose, treat and manage common clinical conditions, chronic diseases, and common neurodevelopmental and behavioral health conditions seen in primary care.
  • Implement tools to manage chronic care in practice and help patients control chronic illnesses, which may include practice transformation and/or quality improvement activities.
  • Demonstrate the ability to communicate effectively with the patient to ensure that diagnosis and treatment recommendations are understood.
  • Recognize when to refer to, or consult with, other specialists to provide optimal patient care.
  • Utilize strategies to help patients overcome barriers to change in relation to chronic diseases, including cultural competency, literacy skills and access to services or care.
  • Educate and counsel patients on behavior modification, strategies to reduce risks of complications, and improve outcomes related to their neurological and/or behavioral health.

The AAFP has reviewed Geriatric Medicine and deemed it acceptable for up to 25.5 Live AAFP Prescribed credits. Term of Approval is from 12/2/2025 to 12/5/2025. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

The AAFP is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

The American Academy of Family Physicians designates this Live activity for a maximum of 25.5 AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

CME activities approved for AAFP credit are recognized by the AOA as equivalent to AOA Category 2 credit.
