3 to 5 years: 2.5 mg per day; increased by 2.5 mg at weekly intervals (not to exceed 0.5 mg per kg per day)
> 6 years: 5 mg three times daily; increase by 5 mg at weekly intervals; maximum dosage of 40 mg per day
