Question 11 of the August 15, 2000, “Clinical Quiz,” pertaining to the article “Cardiovascular Screening of Student Athletes,” is misleading. The first page of the “Preparticipation Physical Evaluation” form includes a question to the student athlete: “Have you ever been told you have a heart murmur?” The second page of the form, on which the examining physician would note abnormal findings, does not include a specific item for recognition of a heart murmur. Although the correct answers remain A, C and D, the question should have emphasized that the information described in choices C and D is not included on the physician's page of the form. The corrected question is reprinted below.
