Translation to Practice (t2p) is an activity supplement available exclusively to all CME provider organizations for activities approved for Prescribed credit through the AAFP Credit System. This activity supplement can be added as a complement to any live, enduring, medical journal, or blended learning* activity approved for AAFP Prescribed credit. t2p is available in both the simple and advanced application pathways. It is not available for the ALSO/BLSO pathway applications.
Translation to Practice allows learners the opportunity to translate learned concepts from a CME activity into their practice and report back to the CME provider organization on the results of their implementation. Learners who choose to participate will be eligible to earn two additional Prescribed credits, per CME activity, upon full completion of the t2p process.
There are three steps associated with participation in t2p:
1. Commitment to Change: After participating in any live, enduring, medical journal, or blended learning* activity approved for AAFP Prescribed credit, a learner may choose to participate in t2p. If the learner chooses to do so, they must complete a commitment to change statement indicating what change they would like to implement.
2. Implement Change: The learner implements the learned change into practice.
3. Post-Activity Evaluation: The learner evaluates whether the implemented change resulted in improved patient care and assesses any barriers or challenges that were encountered.
*Point of Care or Performance Improvement components within a blended learning activity are not eligible for t2p.
CME provider organizations may apply for Translation to Practice by selecting it as a learner evaluation option on the Outcomes Methods question within the session details section of the AAFP Credit System’s online application.
For simple pathway applications, CME providers should select the number of topics within the activity to which t2p will be applied, each is worth an additional two credits. For advanced pathway applications, only one t2p topic should be added per applicable session since each session is entered individually.
Note: CME providers must manage the full three-step t2p process for each t2p topic and will be asked for supporting documentation demonstrating this in the event of an audit.
Once a CME activity is approved for credit, the CME provider organization will be responsible for the administration of the t2p process. Upon approval, CME provider organization will receive a t2p credit statement located in the credit determination letter that may be used for marketing purposes.
t2p must be added at the time of application submission, though if a CME provider organization wishes to add t2p later, they will incur the t2p fee as well as a re-review fee.
Informational materials are provided for reference and include t2p FAQs as well as sample templates for the three-step process. CME provider organizations are not required to use these templates, and retain the flexibility to make changes to them, or create their own templates that align with the three-step process. As with any other CME activity, learners can self-report the credit for participation in t2p once completed. CME provider organizations are responsible for gathering and retaining any data.
